I just watched a scene with Nicole and Sasha Paige and it was distracting the Nicole almost never opened her mouth (except when something was being ...). She smiled only briefly and never showed her teeth. Having had a terrible dental presence myself, I can understand how she may have felt. Probably as you say, her grill is fucked up. Sad; such a pretty girl. I am partial to slim natural girls, and Nicole is a prime example.

It looks like a deformation of her upper jaw, those are usually way more visible and often include a deformation of the upper lip as well.
From what I know that particular part of the face, upper jar under the nose, is the last part of the face that forms and is soft tissue for the longest, therefor it is most likely to have specific types of deformations
Doesn't change the fact that she has a certain aura of attractivity for me, and neither her teeth, nor her recent weight gain (she seems to have diabetes) change that.
And most importantly, she seems to enjoy her work
